Hollister Conform 2™ 25400 is a two-piece closed colostomy pouch with a 55mm coupling, Midi pouch size and beige ComfortWear™ covering, designed for use with a compatible 55mm Conform 2™ skin barrier.

The pouch is intended primarily for people with a colostomy or another stoma producing formed or relatively predictable output, where a closed pouch can be removed and replaced rather than emptied.

Two-piece system

Conform 2™ is a two-piece ostomy system.

The system consists of:

• A separate adhesive skin barrier/baseplate

• A removable ostomy pouch

Unlike a one-piece appliance, the skin barrier can remain attached to the body while the pouch is removed and replaced.

This can be useful for people who need to change their pouch more frequently than their baseplate or who prefer the flexibility of selecting different compatible pouches and barriers.

Reference 25400 is the pouch only. A compatible Conform 2™ skin barrier must be purchased separately if required.

55mm coupling size

The 55mm measurement refers to the mechanical coupling between the pouch and the Conform 2™ skin barrier.

It does not mean that the stoma itself must measure 55mm.

The pouch must be paired with a 55mm Conform 2™ skin barrier.

The opening within the skin barrier is selected separately according to:

• Actual stoma diameter

• Whether a pre-sized or cut-to-fit barrier is required

• Peristomal contours

• Whether flat or convex support is appropriate

A 55mm pouch will not securely connect to a 45mm or 70mm Conform 2™ barrier.

Always match the coupling size of the pouch and baseplate exactly.

Midi pouch size

Reference 25400 is the Midi version of the Conform 2™ closed pouch.

Midi provides a middle pouch capacity within the range, offering a balance between:

Different Conform 2™ closed pouch sizes are available for users who require a smaller Mini or larger Maxi pouch.

Closed pouch

This is a closed ostomy pouch and does not have a drainable outlet.

Instead of opening and emptying the pouch, it is removed from the baseplate and replaced when required.

Closed pouches are generally most appropriate where stoma output is:

• Formed

• Semi-formed

• Relatively predictable

• Produced less frequently

They are therefore commonly used by people with a colostomy.

For frequent liquid output, such as is commonly associated with an ileostomy, a drainable pouch will generally be more practical.

Integrated coupling system

Conform 2™ pouches incorporate an integrated mechanical coupling system.

When the pouch is correctly connected to the compatible skin barrier, audible clicks can be heard as the coupling locks into place.

This provides additional reassurance that the pouch is properly attached.

After fitting the pouch, check around the complete coupling to confirm that:

• The pouch is attached evenly

• The coupling is completely engaged

• There are no sections that remain unlocked

• The pouch cannot easily separate from the barrier

AF300™ filter

The pouch incorporates Hollister’s AF300™ filter.

Gas generated by the bowel can build up inside an ostomy pouch and cause ballooning.

The filter is designed to:

• Allow gas to escape gradually

• Help reduce pouch inflation

• Filter odour before gas is released

• Help maintain a flatter pouch profile

• Improve discretion during wear

The filter should not be punctured, cut or otherwise modified.

ComfortWear™ pouch panels

The beige pouch incorporates soft ComfortWear™ panels.

These provide a softer surface between the pouch and the body and help make the appliance more comfortable and discreet during normal wear.

The beige covering also conceals the pouch contents.

Flat or convex barrier compatibility

The Conform 2™ pouch can be used with an appropriate compatible flat or convex Conform 2™ skin barrier, provided the coupling size is 55mm.

Flat barriers are commonly suitable where the stoma protrudes adequately and the surrounding abdominal surface allows a reliable seal.

Convex barriers may be considered for stomas that are:

• Flush with the skin

• Retracted

• Situated within dips or folds

• Difficult to seal using a flat barrier

Convexity should be selected following an appropriate stoma assessment, particularly if convex products have not previously been used.

Changing the pouch:

Wash and dry your hands.

Prepare a new Conform 2™ closed pouch.

Check that the existing skin barrier remains securely attached and that there are no signs of leakage beneath it.

Gently separate the used pouch from the coupling according to the manufacturer’s instructions.

Dispose of the used closed pouch appropriately.

Check the exposed baseplate and coupling.

Position the new pouch over the 55mm coupling ring.

Press the pouch and baseplate coupling together around the complete circumference.

Listen and feel for the audible clicks as the coupling engages.

Check around the entire coupling to ensure the pouch is completely attached.

Gently check that the pouch is secure before repositioning clothing.

When to change the pouch:

Closed pouches should be changed according to individual output and personal requirements.

Do not allow the pouch to become excessively full.

Changing before the pouch becomes heavy can help:

• Reduce pulling on the baseplate

• Improve discretion

• Reduce pressure on the coupling

• Reduce the risk of accidental detachment

• Improve comfort

The skin barrier does not necessarily need to be changed every time the closed pouch is replaced if it remains secure, clean and clinically appropriate to leave in place.

Changing the skin barrier:

The Conform 2™ baseplate should be changed according to the manufacturer’s instructions and the individual’s normal stoma-care routine.

Change the barrier sooner if:

• Leakage occurs

• The adhesive begins lifting

• Output reaches beneath the barrier

• Peristomal skin becomes sore

• The barrier becomes damaged

• The appliance no longer feels secure

Persistent leakage should not simply be managed by repeatedly replacing the pouch. The baseplate size, stoma opening and overall appliance fit may require reassessment.
